Completed deed of surety form number 3-1/0005 signed by bank or employer.
Copy of passport (personal data page).
2 passport size photos.
Police clearance certificate from country of origin.
Latest Police Clearance from Namibia.
Marriage Certificate (if married) (Divorce letter).
Death Certificate of late spouse (all widows and widowers).
Detailed CV.

Questionnaire (training and experience).
Upon approval, applicant will be requested to submit copy of approval
letter, passport together with original receipt of payment for endorsement
of permit.
A non-refundable N$80.00 handling fee is payable upon submission of
application.
