Republic, containing the Resolution No. 135 (this document must be supplied
by the Dominican company).
Certificate of good conduct (apostilled from the Foreign Office).
A letter from doctor/GP stating that you are in good health.
Most recent bank statement of the bank account of the person funding the
journey or proof of funds.
Application form and a letter with details of your request.
The visa fee: £450.00, (600 USD), non-refundable, payable at the time of
application, either in cash, postal order or banker’s draft. We do not
accept personal cheques.
